The following form is a sign up for a Creative Centre Class available for 10 matches. Please note that signups will be on a first come first served basis but we will keep a waitlist in case a spot opens up. A mentoring coordinator will be in touch with you shortly to confirm whether you have a spot or you are on the waitlist and provide you with any other details necessary.
